No contemporary philosopher has argued more consistenily or more convincingly for a God of becoming than Charles Hartshorne. Boehme looms largein the historical background of his dipolar theology: both classical theism, which sees God as supreme actuality and most strictly absolute, and pantheism, whichsees in God only supreme potentiality and universal relativity, are correlated in his panentheism. The ultimate contraries are united in the divine relativity,where eternal permanence and temporal process are both preserved in a tension that, logically, precedes them.Hartshorne (...) has been reluctant to develop relationships to earlier and "simpler" representatives of his type of philosophical theology. Berdyaev, on the other hand, is ready to acknowledge Boehme as the real founder of his own philosophy of freedom. The affinity between the two stands out sharply when they explore the problem of evil, tragedy within divinity, or the Unground as Nothingness which gives rise to Something and the entire theogonic process. Berdyaev sees Boehme as one of the rare thinkers who broke with the optimistic rationalism of Western thought to construct a more spiritual philosophy of tragedy.Heidegger's revolt against Western ontology and Christian theology has certain affinities to Boehme's thought. We may point to but one aspect here, the central basic question of metaphysics, which is foolishness to Christian theology: "Why is there any being at all and not rather nothing?" This question and Heidegger's "Nothingness" have obvious connections with Boehme's question and his "Nichts." For both. Nothingness is a primordial and fundamental working. It is found in Being itself rather than outside it. It gives rise through primordial discord to Being. To be sure, Heidegger develops his dialectic far beyond anything suggested by Boehme when he interprets his Dasein as suspended in dread over das Nichts. But Heidegger's profoundly dialectical conception of Being has obvious relations to the non-Being of Boehme: Nothingness or nihilation is present in all beings and is the essence of Being itself. It is the dynamic power of Being thai gives rise to Being. Self-negation is always present in the coming-into-presence of Being. For both Boehme and Heidegger Nothingness is required for the "letting-be" of beings, for the un-concealment of Being. (shrink)

In Quarles' world the emblem as traditionally conceived must strain across a widening gap between the verbal and the visual. Rosemary Freeman's criticism of Quarles, that in a mechanical "imposition of meaning" the text of the emblem applies an interpretation to, rather than discovers a significance within, the image, is more apt than Freeman realized. With the semantic congruence between word and image no longer guaranteed, artists attempting to yoke the two would have to reconceive the relationship between them. Seen (...) as a response to this need, Blake's illuminated books complicate the emblem tradition in an art of dazzling improvisatory juxtapositions. Indeed, his revaluation of the ties between "body" and "soul" may be taken in one sense as a revision of the emblematist's traditional distinction. Words, once the soul of the emblem, now become truly animate for Blake - flowing, sprouting, multicolored - while their quirky energy, no longer restrained by standardized print, is embodied on sensual, quasi-pictorial shapes; images speak in a new and private vocabulary of emblematic birds, curling tendrils, and other forms that gesture allusively from plate to plate. These frame, underscore, celebrate, intrude upon, parody, or oppose themselves by "contraries" to the meaning of the adjoining text. If Quarles' work signals the failure of the emblem in England, its success in probing the problems of combining language and imagery points toward the renewal of the form in Blake. Ernest B. Public health care interventions—regarding vaccination, obesity, and HIV, for example—standardly take the form of information dissemination across a community. But information networks can vary importantly between different ethnic communities, as can levels of trust in information from different sources. We use data from the Greater Pittsburgh Random Household Health Survey to construct models of information networks for White and Black communities--models which reflect the degree of information contact between individuals, with degrees of trust in information from various sources correlated with (...) positions in that social network. With simple assumptions regarding belief change and social reinforcement, we use those modeled networks to build dynamic agent-based models of how information can be expected to flow and how beliefs can be expected to change across each community. With contrasting information from governmental and religious sources, the results show importantly different dynamic patterns of belief polarization within the two communities. (shrink)

Background: Recent research demonstrates that people sometimes make different medical decisions for others than they would make for themselves. This finding is particularly relevant to end-of-life decisions, which are often made by surrogates and require a trade-off between prolonging life and maintaining quality of life. We examine the impact of decision role, patient age, decision maker age and multiple individual differences on these treatment decisions. Methods: Participants read a scenario about a terminally ill cancer patient faced with a choice between (...) an aggressive chemotherapy regimen that will extend life by two years and palliative treatments to control discomfort for one remaining month. Participants were randomly assigned to one of three decision roles (patient, physician, or an abstract other) and the scenario randomly varied whether the patient was described as 25 or 65-years old. Results: When deciding for a 65-year old patient, approximately 60% of participants selected aggressive chemotherapy regardless of decision role. When deciding for a 25-year old patient, however, participants were more likely to select chemotherapy for a patient (physician role) or another person (abstract other) than for themselves (70%, 67%, and 59%, respectively). In addition, confidence that powerful others (eg, physicians) control one’s health, as well as respondents’ age and race, consistently predicted treatment choices. Conclusions: Patient age appears to influence medical decisions made for others but not those that we make for ourselves. These findings may help to explain the discord that often occurs when younger cancer patients refuse life-extending treatments. (shrink)

This study examined female sex workers’ evaluation of ethically relevant experiences of participating in an HPV4 vaccine clinical trial conducted in Lima, Peru. The Sunflower Study provided all participants with HPV testing, treatment for those testing positive, and access to the vaccine for all testing negative. Themes that emerged from content analysis of interviews with 16 former participants included the importance of respectful treatment and access to healthcare not otherwise available and concerns about privacy protections, the potential for HIV stigma, (...) and poststudy abandonment. This study sought to assess barriers and enhance readiness to consent to home and Planned Parenthood HIV testing among 60 out-patients from a mental health and substance abuse clinic in rural Appalachia. Testing barriers included not knowing where to get tested, lack of confidentiality, and loss of partners if one tested sero-positive. The intervention yielded lowered HIV stigma, increase in HIV knowledge, and agreement to take the HIV home test. These results are encouraging because they suggest that a brief educational (...) intervention is a critical pathway to the success of the National Institutes on Drug Abuse’s Seek, Test, Treat, and Retain initiative in poor rural counties. This study documents how people who inject drugs in rural Puerto Rico perceive payments for participating in HIV epidemiological studies. In-depth interviews were conducted among a subset of active PWID older than 18 years of age who had been previously enrolled in a much larger study. Findings suggest that financial compensation was the main motivation for initially enrolling in the parent study. Then, as trust in the researchers developed, participants came to perceive compensation as part of a reciprocal exchange in (...) which they assisted researchers by providing a trustful account of their experiences and researchers reciprocated with financial support. United States federal regulations for pediatric research with no prospect of direct benefit restrict institutional review board (IRB) approval to procedures presenting: 1) no more than "minimal risk" (§ 45CFR46.404); or 2) no more than a "minor increase over minimal risk" if the research is commensurate with the subjects' previous or expected experiences and intended to gain vitally important information about the child's disorder or condition (§ 45CFR46.406) (DHHS 2001). During the 25 years since their adoption, these regulations have helped (...) IRBs balance subject protections with the pursuit of scientific knowledge to advance children's welfare. At the same time, inconsistency in IRB application of these regulations to pediatric protocols has been widespread, in part because of the ambiguity of the regulatory language. During the past decade, three federally-charged committees have addressed these ambiguities: 1) the National Human Research Protections Advisory Committee (NHRPAC) (Washington, DC), 2) the Institute of Medicine (IOM) Committee on the Ethical Conduct of Clinical Research Involving Children (Washington, DC); and 3) the United States Department of Health and Human Services Secretary's Advisory Committee for Human Research Protections (SACHRP) (Washington, DC). The committees have reached similar conclusions on interpretation of language within regulations § § 45CFR46.404 and 406; these conclusions are remarkably consistent with recent international recommendations and those of the original National Commission for the Protection of Human Subjects of Biomedical and Behavioral Research (1977) report from which current regulations are based. Drawing on the committees' public reports, this article identifies the ethical issues posed by ambiguities in regulatory language, summarizes the committees' deliberations, and calls for a national consensus on recommended criteria. Drawing on a conception of scientists and community members as partners in the construction of ethically responsible research practices, this article urges investigators to seek the perspectives of teenagers and parents in evaluating the personal and political costs and benefits of research on adolescent risk behaviors. Content analysis of focus group discussions involving over 100 parents and teenagers from diverse ethnic and socioeconomic backgrounds revealed community opinions regarding the scientific merit, social value, racial bias, and participant and group harms and (...) benefits associated with surveys, informant reports, intervention studies, blood sampling, and genetic research on youth problems. Although there is clearly a need for evidenced‐based behavioral or biomedical prevention or treatment programs for suicide, substance abuse, and sexual health targeted to members of the LGBT population under the age of eighteen, few such programs exist, due in substantial part to limited research knowledge. Ambiguities in regulations that govern human subjects protections and the related inconsistencies in institutional review board (IRB) interpretations of regulatory language are the key reason for the lack of rigorous clinical trial evidence to support (...) treatment choices and prevention approaches to reducing health disparities for this population. Given the socially sensitive nature of suicide, substance abuse, and HIV and STI research in general and LGBT research specifically, in the absence of empirical data to guide their decisions, IRBs must often rely on subjective judgments of minimal risk, which can lead to overestimation of the magnitude and probability of psychological, social, and informational harms that might arise from LGBT youth participation in clinical trials. In addition, more than other youth, LGBT adolescents whose families are unaware of their sexual orientation or gender identity or whose families have victimized them on account of it may be reluctant to participate in studies that require guardian permission. This, in turn, intensifies problems of recruitment and unbiased sampling. However, many IRBs are reluctant to apply federal regulations permitting waiver of guardian permission under conditions in which such permission is clearly not “feasible” or “reasonable” to require. Consequently, many investigators have excluded LGBT individuals under eighteen years of age in health intervention research proposals because of anticipated or actual difficulties obtaining IRB approval. This situation is in conflict with current ethical discourse focusing on the right of youths to participate in trials that will protect them from receiving developmentally untested, inappropriate, and unsafe treatments. In this article, we describe these barriers and recommendations for providing LGBT youth safe and fair access to health research. (shrink)

ABSTRACT Online research has become a critical recruitment modality for understanding and reducing health disparities among hidden populations most at risk for HIV infection. There is a lack of consensus and guidelines for the responsible conduct of online recruitment for HIV risk populations. Using semi-structured phone interviews, this study drew on the experiences of principal investigators engaged in online HIV research to illuminate scientific and ethical benefits and challenges of social media recruitment. Using Thematic Analysis five major themes emerged: sampling (...) advantages and disadvantages; challenges of data integrity; control of privacy protections; researcher competence and responsibility; and resources.
